Low Anterior Resection with EEA Stapler for Rectal Cancer


Abstract
19 Patients of rectal cancer from April 1985 to December 1987 at Chosun University hospital had low anterior resection with EEA stapler.
Clinical results was as follows;
1) Most prevalent age group was 4th and 5th decade (63.2%) . The ratio of male to female was 1.1:1.
2) The most common site of the tumor was midrectal region(4-8cm from the anal verge) and 12 cases(63.2%) were in this location.
3) The duration of symptoms and signs prior to admission was less than 3 months (36.8% ) in 7 cases.
4) The most common complaints were gross blood in stool in 14 cases (73.3%) and Change in bowel habits in 11 cases(57.9%).
5) In diagnosis, diagnostic approach was made by sigmoidoscopy with biopsy in 15 cases (89.5%) and rectal examination in 13 cases (68.4%) , barium study, colonoscopy, computerized tomography and IVP with cystoscopy.
6) End to end anastomosis was performed in 17 cases ( 89.5%) and side to end anastomosis was in 2 cases (10.5%) .
7) The resected margin below the distal margin of the tumor was as follows ;
4 cases (21.1%) in 1-2cm, 13 cases (68.4%) in 3-4cm and 2 cases (10.5%) in above 5cm.
8) Adenocarcinoma was the most common histological type in 18 cases (94.7%) villous
adenoma was 1 case.
9) Distribution of patients according to modified Dukes classification was as follows ;
Dukes¢¥ A(10.5%), Dukes¢¥ B(42.1%), Dukes¢¥ C(26.3%) and Dukes¢¥ D(21.1%).
10) The mostcommon postoperative complication was noted in 2 cases (10.5%) which were anastomotic leakage with local abscess formation.
11) The distant metastasis was liver in 2 cases (10.5%) and pelvic organ in 2 cases. 12) The relation between local recurrence and distal rectal margin was as follows ; 2 cases (10.5%) in 1-2cm, 1 case (5.3%) in 3 -4cm.
